Facing challenging economic  conditions, healthcare institutions have increasingly turned to philanthropy to combat declining overall revenues and help bridge a period of uncertainty with the advent of the Affordable Care Act (ACA).

Traditionally healthcare foundations kick off their fundraising with a special event, which acts as the main driver of their philanthropic revenue. As organizations pay more attention to increasing revenue sources, many have started focusing on enhancing and building major gift programs. The goal is to diversify donation sources and decrease reliance on special events.

There were 98 mergers and acquisitions (M&A) among hospitals and health systems in 2013, up 3 percent from 2012, and more than 50 percent from 2010, according to a survey by Kaufman Hall. According to the Healthcare M&A Watch, a Modern Healthcare report, the deal volume in hospital mergers has shrunk while the value per deal has risen in the first quarter of 2014.

The health care market has been going through considerable changes including many physicians shifting from having their own private practice to salaried jobs. According to the American Medical Association (AMA) about 60 percent of family doctors and pediatricians, 50 percent of surgeons and 25 percent of surgical subspecialists are  already employees rather than independents. That number is predicted to grow.
